CPS / Adoption Evaluation Consent

This psychiatric evaluation is conducted to assess the applicant’s emotional, psychological, and behavioral functioning in relation to suitability for foster care, guardianship, or adoption placement.

This evaluation may include:
Clinical interview
Mental health history review
Trauma and stress assessment
Parenting readiness evaluation
Substance use screening
Emotional regulation assessment
Collateral information (when authorized)

I understand that this psychiatric evaluation is being completed as part of a CPS/adoption review process. I understand that information gathered may be included in a written report shared with authorized agencies involved in placement decisions.
